Community Health Care Report
High-Risk & Underserved Populations
Rockford Wellness Foundation serves low-income, uninsured, and medically underserved residents across Northern Illinois and the greater Rockford metropolitan area. Local health assessment data indicates that while acute emergency care is available, essential restorative, supportive, and root-cause health therapies—such as acupuncture, chiropractic care, red light therapy, infrared sauna detox, and targeted nutritional guidance—are rarely covered by Medicaid or traditional commercial insurance. This creates severe out-of-pocket financial barriers for disadvantaged households suffering from chronic pain and complex conditions.
To increase healthcare accessibility, the Foundation established a Board-approved 4-tiered Sliding Fee Schedule tied directly to Federal Poverty Guidelines (FPG), offering 50% to 100% treatment subsidies for eligible local residents to ensure cost is never a barrier to receiving comprehensive, whole-person care.
Strategies for Gathering Community Input
The Foundation utilizes a multi-channel feedback system to ensure program development directly reflects community needs:
-
Patient & Client Surveys: Regular post-care evaluations and health literacy feedback forms collected from individuals receiving subsidized care.
-
Community Advisory Meetings: Bi-annual listening sessions with local faith leaders, social service agency coordinators, and community advocates representing low-income neighborhoods in Rockford.
-
Clinical Case Management Reviews: Ongoing input from clinical staff and community outreach coordinators to adapt service modalities based on emerging public health concerns in Northern Illinois.
Partnerships with Providers & Community Organizations
To promote continuity of care and maximize cost-efficiency, Rockford Wellness Foundation maintains key collaborative linkages:

-
Facility & Resource Sharing: Formal administrative and facility coordination agreements (including shared clinical space with Midwest Workforce, PLLC), ensuring 100% of grant funding goes directly toward patient care subsidies and educational tools rather than administrative overhead.
-
Safety-Net Referrals: Cross-referral partnerships with regional safety-net clinics, community health hubs, and local non-profits to transition patients needing non-pharmacological pain management and restorative wellness into our subsidized care plans.
Community-Oriented Program Example: The Wellspring Fund
The Wellspring Fund serves as the public-facing patient financial assistance arm of our governed Healing Access Program, delivering structured 120-day Intensive Care Plans and patient health literacy workbooks to low-income individuals.
-
Number of Clients Served: Target cohort of 20 intensive care pilot patients and 50 health literacy workshop participants.
-
Total Program Budget: $25,000.
-
Percent of Agency Budget: 20% of total annual Foundation operating budget.
-
Percent Reimbursed by Third-Party Payers: 0% (these non-pharmacological root-cause therapies are not covered by traditional insurance or Medicaid).
-
Percent Covered by Grants/Donations: 100% of patient subsidies are funded through private/public grant funding and Foundation philanthropy.
